- How to get started: Complete the Guardian Angel Enrollment Form and one Pet Profile Form for each animal, then email them or mail printed copies to SpokAnimal Guardian Angel Program, 710 N Napa Street, Spokane, WA 99202. To name SpokAnimal as caretaker, include a statement in your will placing your pets in SpokAnimal's custody.
